HIPAA Statement
Our Commitment
Salem Health Solutions is committed to maintaining the privacy
and security of our customers' information. Collecting the minimum information
necessary to provide our services and using information only as authorized
by our customers is foremost in every facet of our business. Personally
identifiable information will not be shared at any time outside of our business
relationship with a customer, except as allowed or required by state and
federal law, court orders, and regulatory and accrediting bodies.
HIPAA
Salem Health Solutions serves as an extension, or business
associate, of our customers who are covered entities under the Health Insurance
Portability and Accountability Act of 1996 (HIPAA). In the course of performing
data warehousing, aggregation, analysis, and reporting functions, Salem
Health Solutions may receive data from our customers that contain protected
health information (PHI). PHI is individually identifiable health information
related to an individual's past, present or future health condition or to
the provision, or payment for provision, of health care to the individual.
HIPAA establishes regulatory requirements related to PHI.
It is the policy of Salem Health Solutions to ensure the
security and confidentiality of all data provided to it by contracted entities,
and to abide by all contractual obligations, laws and regulations governing
such actions. Salem Health Solutions has developed policies, procedures
and processes to ensure compliance. In addition, Salem Health Solutions'
contracts with our customers govern the use and disclosure of PHI.
Safeguarding Protected Health Information
Salem Health Solutions makes all reasonable efforts to safeguard
PHI from intentional or unintentional use or disclosure. Accordingly, Salem
Health Solutions has policies, procedures and systems in place to protect
the transmission of, limit access to and govern the use and disclosure of
PHI. In addition, Salem Health Solutions has extensive privacy and security
training programs that are required for all workforce members.
Salem Health Solutions has physical safeguards to protect
computer systems, buildings and equipment from fire and other environmental
hazards, as well as intrusion and catastrophic failure. All Salem Health
Solutions servers reside in a secure and environmentally protected facility.
Salem Health Solutions stores customers' PHI in secure computer
systems with strict access controls. Salem Health Solutions has identified
the members of its workforce who require access to PHI to carry out their
company duties and has restricted access to only those categories of PHI
necessary to perform those duties.
Use of De-identified Information
SHS may use de-identified health information or summarized data for business-related
purposes including development of comparative databases. This data may
be valuable for research, quality improvement and measurement purposes.
Consequently SHS may release de-identified information externally after
ensuring that it is not possible to implicitly or explicitly identify
an individual. |
